Please provide data sufficient to show the following for each Award or Commendation, including those that were denied, that exists in a machine-readable format (e.g., spreadsheets): ● Award ID/reference number ● Any associated file numbers (e.g., CB, RD, etc.) ● Award or commendation type ● Award or commendation request date ● Current status ● Award incident ● Reason for delay ● Incident start date ● Incident end date ● Ceremony date ● Requester5 of award or commendation ○ First name ○ Middle name or initial ○ Last name ○ Birth year ○ Gender ○ Race ○ Star number3 ○ Appointment date ○ Rank4 ● For each officer5 receiving the award or commendation: ○ First name ○ Middle name or initial ○ Last name ○ Birth year ○ Gender ○ Race ○ Star number3 ○ Appointment date ○ Rank4 ○ Last promotion date, if any
